You will need the following documents for live scan fingerprinting:
- Acceptable valid photo ID
- BCII 8016 “request for live scan fingerprints” service form
- Social security number

Mandatory fields that are required to be completed are:
- Your social security number
- Your driver’s license number
- Contact information, name of the agency contact person
- Telephone, agency’s contact person telephone number
- ORI Code – The agency’s “applicant record identifier” assigned by the DOJ
- Type of application & job title
- Agency address that will receive your applicant background information
- Mail code the agency’s 5-digit DOJ assigned code
- Your information, name and description
- Referring organization’s billing number ( if they are paying for your background check fees )
- Your phone number
- DOJ and / or FBI level of service selection
It is mandatory to include your social security number on your BCII 8016 document. DOJ and FBI mandatory requirement. If you refuse to include your social security number, service will be denied for live scan fingerprint. A valid photo ID is also mandatory for live scan fingerprinting requests.
The following are examples of acceptable forms of photo identification:

- Alien Registration / Immigration Green Card
